About Intake

How do you find out about us? 

  • General Practitioner with completed mental health plan and referral.

  • Enquire through our website, via phone, social media, or word of mouth.

  • By an agency or school you are linked in with.

  • NDIS service providers.

What do I need to make a referral?

Firstly, give us a ring on (02) 6013 9757 to discuss your options and complete an intake. 

  • For a Medicare rebate: we need a mental health care plan and a referral from your GP.

  • Funded by NDIS: Email us a copy of your NDIS plan.

  • Other agencies: Please email admin@phoenixwingswellness.com.au, your client will still need to ring to make intake, but we have a professionals referral form to fill out too please!

  • Private pay - just give us a ring!

Intake and Consent Forms

Reception will ask you a few questions like what’s the current issues happening for you or your child/adolescent/young person. They will email you a consent form that needs to be signed online before our team can be assess your situation and place the client on the waitlist accordingly. We will contact you when an appointment becomes available.

First Appointment

We will contact you when an appointment becomes available, and a reminder text message will be sent 48 hours beforehand (and email 5 days prior!). You will need to reply yes to confirm. 

During the initial appointment, assessments will be completed in order to create a treatment plan that fits you best. 

If you feel Phoenix Wings Wellness is not a good fit after the initial appointment, we will support you to find the right practitioner.
